Patient Information Practice Hours Our Cleveland office ours are Monday-Friday from 8:00am to 5:00pm with some Saturday hours available. We make every effort to schedule you so that your wait time is minimal. However, the physicians schedule can sometimes fluctuate due to emergency surgery. What To Bring To Your First Visit Before your first visit with TN Valley Bone and Joint, please download, print and complete the forms provided below. Please bring completed forms to your scheduled appointment along with Medical records pertaining to visit, Diagnostic images(Xray), prescriptions and Insurance cards. Out of Network Benefits welcome. Medication Policy In an effort to assure all your medication needs are addressed at the time of your office visit: Please discuss medication refills or changes with your provider before leaving the room. Please bring all medications which you are currently taking to each visit. Medication Refills: Medication should only be taken as prescribed. Early refills will not be authorized. Medications will only be refilled or changed at the time of your scheduled visit. Medications will not be refilled by phone. Medication replacement: Lost or stolen medications will not be replaced. After Hours Process If you have a life threatening emergency, please dial 911. Otherwise, call the hospital or the on-call doctor will assist you with your needs.
