When making an appointment, it is important to have the following information available:
- If you are requesting a particular provider and/or clinic
- Nature of your appointment need…If you are having multiple orthopedic problems or a specific problem, be sure and tell the Appointment Clerk, as this helps to determine the best time period for your appointment.
- If you are new or existing….If you have been seen by Dr. Hutcheson or a provider of TN Valley Bone and Joint in the past ten years, you are considered an “existing patient”, and your medical records are usually available for reference. If you have never been seen by one of our providers, you are considered a “new patient”.
- Insurance information….. Providing accurate, up to date information when making appointments allows the staff to expedite the delivery of your care. Always have your card in front of you when you call for appointments.
- Calendar…..While every effort is made to accommodate your preferred date and time for an appointment, you may have to be seen on an alternate date, time or clinic.
{If you are having an emergency or life-threatening situation, you should go immediately and directly to the nearest Emergency Room or call 911.}
While the Appointment Schedulers make every effort to accurately communicate times, dates and locations, it is helpful for you to write down this information and confirm it before you complete your conversation with the Scheduler.
What To Bring To Your First Visit
- Medical Records and or Diagnostic Images (X-rays and/or MRI)….. These records are very important and allow the providers of TN Valley Bone and Joint to understand the full extent of your injury or problem. It is the patient’s responsibility to see that the requested medical records are available at appointment time.
- Medications…..In an effort to assure all your medication needs are addressed at the time of your office visit. Please discuss medication refills or changes with your provider before leaving the room. Please bring all medications which you are currently taking to each visit.
- Insurance Cards…..We will copy your cards upon arrival for your appointment.
- Patient Forms…..Please print and complete (Patient Registration, Patient History Form, and HIPAA)
